Research Requests for Expired or Lost Tickets

IMPORTANT NOTICE

MISS DIG 811 requires a subpoena to release ticket information to anyone other than the original requester/company or a member utility.

In accordance with Section 6 (4) of Public Act 174 of 2013, MISS DIG 811 maintains an archive of locate requests (tickets) for six years. For a fee, MISS DIG 811 will search these archived requests and provide the requested information. When completing this form, please include as much detail as possible about the work location. If some information is missing, research may still be conducted as long as the work location is clearly identified. Please do not assume that MISS DIG 811 can determine the correct city, village, township, or county from partial address information.

Research Request fees are assessed per job and cover the cost of preparing the documents. For $96.30, you will receive a copy of the ticket and, upon request, a voice recording of the ticket (if it was placed over the phone). Please allow one to two weeks for processing after payment is received.
Note: Voice recordings prior to 2022 are unavailable.

If you or your organization is a MISS DIG 811 member, you may access most ticket and Positive Response data associated with your membership at no additional cost. This excludes data requested through a legal subpoena and voice recordings. If you're unsure whether your request qualifies for free access, please contact our Member Services Department at (800) 482-7161 or membersupport@missdig811.org before completing this form.

Not sure if you're a member? Members have completed onboarding paperwork and receive an annual membership invoice.

For a prompt response, please provide as much detail as possible on this form. For large or complex Research Requests (e.g., multiple locations), contact the MISS DIG 811 Administration Office at (248) 370-6400.
